Welcome to Patient Center. Here you’ll find access to the Patient Portal Health Record (PHR) and forms that can be printed and completed prior to your visit. To schedule an appointment please contact us at (757) 564-1907 or via email (info@williamsburgeye.com). Please provide a phone number or email address where you may be reached along with your preference of time and day. We will notify you of our next available appointment and wait for your confirmation.

Patient Health Record

​​​​​​​Within the Patient Health Record portal you’ll be able to:

• Update key information such as an address, phone number, email address, and more.

• Complete a pre-examination interview online.

• Check the status of eyeglasses and contact lens orders and prescriptions.

• Access your billing statement(s).

To access the secure patient portal click on the Patient Health Record button below. Your username will be your first initial, last name, and the last two digits of your birth year. For example, Jsmith07.

Note: Usernames are required to be at least 8 characters long.

Our office will provide a temporary password to you. If this is your first time logging into your PHR and you have not received your temporary password please call our office at (757) 564-1907.

We do have the ability to place individual accounts under a family account. Please contact our office for further details.
